Need Assessment

This role will determine whether the applicant is an appropriate candidate for a SAWs Virginia ramp, based on observable medical disabilities, circumstances and assets for preliminary approval. 

SUPERVISED AND SUPPORTED BY:  Executive Director

 

DUTIES:

  • Receive an evaluation request by email from BuilderTrend, the Project Management System. 

  • Call the applicant to obtain any pertinent information not given on the application. 

  • Call applicant to schedule a day and time for the visit.

  • At the client's home, present the Client Release and Waiver of Liability permission form for applicant to sign. 

  • If applicant is a renter or resides within an HOA, present the applicant the Landlord/HOA Permission to Build Rampform for the applicant to acquire the appropriate signatures. (In some cases, you may need to help the applicant find a way to get the form to the landlord or HOA manager. Primarily, it's the client's responsibility.)

  • By observation assess the applicant’s physical impairments and limitations that a ramp would be an appropriate solution.

  • By observation assess whether the person has any significant or disabling cognitive challenges.

  • By observation assess whether the applicant truly cannot afford a ramp.

  • Inform the applicant of typical wait time to obtain a ramp.

  • Advise the applicant the next SAWs contact will be a survey team to schedule a date and time to survey the ramp location and that they will need to be present.

  • Email the medical assessment that justifies the need for a ramp, plus a photo of the permission form to the Executive Director for recording. 

  • All forms must be on record before final approval can be given.

 

QUALIFICATIONS:

  • Experience in a medical practice of general medicine or,

  • Experience in a medical practice in geriatric medicine or,

  • Experience in a medical practice of orthopedic medicine or,

  • Experience in the field of physical therapy.

TIME COMMITMENT REQUIRED:

  • This may be a certain number of hours per week or month, or it may be a specific day and time based on your and client’s schedule.

  • Location is determined by the location of the applicant.
